Telemedicine Service Guidelines & Patient Consent Overview: This document sets forth the guidelines for our telemedicine services, including prescription policies, informed consent, service termination rights, and insurance documentation protocols. By using our services, patients agree to these terms. Prescription Policy: Controlled Substances: The prescription of controlled substances, such as opiates, Adderall/amphetamines, benzodiazepines, pregabalin, and testosterone, is strictly prohibited. Regulatory Compliance: We comply with state-specific telemedicine regulations, which may limit the availability of certain medications. Medication List: An up-to-date list of non-prescribable medications through our service is available to patients. Informed Consent: Service Delivery: Telemedicine involves healthcare delivery via electronic communication. Limitations: While convenient, telemedicine is not a substitute for all medical consultations, especially those requiring physical exams or urgent care. Privacy: We adhere to HIPAA standards to ensure patient privacy and data security. Service Agreement: Consent: Using our services implies agreement to these terms. Sick notes are valid for up to 72 hours, with a recommendation for a physical assessment by a local provider for longer absences. Extended Sick Notes: Follow-up Requirement: Conditions warranting absences beyond 72 hours will include a directive for in-person evaluation, ensuring comprehensive care.
