Our home care visit service, available exclusively in the Miami area, is meticulously designed to cater to patients who require a personalized health assessment outside the scope of telemedicine. This service, performed by a qualified nurse practitioner, spans a concise 30-minute session, ensuring focused and efficient healthcare delivery directly in the comfort of your home. Service Overview: Personalized Assessment: Conducted in-person by a nurse practitioner, focusing on evaluating specific health concerns that are not suitable for telemedicine. Interventions and Prescriptions: Immediate healthcare interventions and prescriptions will be provided as needed, based on the assessment. Non-Emergency Care: This service is strictly for non-emergency situations. Patients experiencing urgent symptoms such as chest pain, shortness of breath, altered mental status, syncope, or loss of consciousness should seek emergency medical care immediately. Key Policies and Considerations: Scope of Service: While our home visit offers a comprehensive evaluation, it does not replace the ongoing care provided by a regular healthcare provider or substitute the extensive care available at physical healthcare facilities. Continuity of Care: We emphasize that this service should complement, not replace, the care from your existing healthcare provider. It is crucial for patients to maintain their relationship and continuity of care with their primary healthcare providers. Geographical Limitation: Currently, this service is exclusively offered to residents within the Miami area, ensuring timely and accessible care to our community. Duration and Efficiency: Each visit is streamlined to a 30-minute window to address health concerns efficiently, making every minute count. Additional Services Provided: Health Education: Patients will receive tailored health education, focusing on disease prevention, health maintenance, and lifestyle modifications. Follow-Up Recommendations: Based on the assessment, recommendations for follow-up care with your primary healthcare provider or specialists will be provided. Documentation and Reporting: A detailed report of the visit will be shared with your primary healthcare provider to ensure seamless integration of care. Cancellation Policy

Telemedicine Subscription Policy – Direct Primary Care of Coconut Grove Provider: Camilo Murillo, APRN, FNP-BC Membership Overview: Annual membership fee is a one-time, non-refundable yearly fee. Includes up to three (3) telemedicine visits per month via secure video or phone. Medications, labs, imaging, and specialist care are not included. Medication Policy: No controlled substances will be prescribed or renewed. Prescriptions may only be provided for the subscribing member if clinically appropriate. Membership will be terminated without refund for drug diversion, misuse, or attempts to obtain medications for another person. Member Agreement: Membership is for one patient only, non-transferable, and non-refundable. Unused visits do not roll over. Members are responsible for all pharmacy charges, laboratory/imaging/diagnostic testing, and external specialist/urgent care/hospital services. Important Limitations: This program is not insurance and does not replace comprehensive health coverage or a physical examination with a local primary care provider. Membership is for routine telemedicine consultations only. Emergencies must be directed to 911 or the nearest ER. Provider Commitment: We provide secure, timely telemedicine consultations, education on diagnoses/medications/self-care, referrals to in-person or specialist evaluation when appropriate, and maintain proper documentation. Termination of Membership: Membership may be cancelled at any time without refund if drug diversion or misuse is suspected, controlled substances are requested, excessive medications or unnecessary referrals are demanded, patient care needs exceed telemedicine scope, or program policies are not followed. Final Disclaimer: This membership is for expanded access to telemedicine primary care only. It is not insurance, does not replace in-person physical exams, and does not substitute for a local primary care provider to coordinate long-term health needs.
